To release a pet to Lewisville Animal Services, please fill out the form below.

Requirements:

• Proof of residency of Lewisville. Proof of residency can be in the form of a driver’s license, current water/electricity bill, or a housing lease agreement.
  • This requirement does not apply to owners who are wishing to return a pet that was adopted from Lewisville Animal Services.
• Valid Government Identification (driver's license, passport, etc.)
• Releasing person must be 18 years or older.
• If releasing multiple pets, each pet must have a new owner release form submitted.

Once Owner Release Form is Submitted:

• Lewisville Animal Services will contact the owner releasing the pet on or before the next business day to schedule a drop-off appointment. Appointment hours are Tuesday through Friday, 10 a.m. - noon.
• Pets must be leashed or in a carrier when arriving to the drop-off appointment.
• Please bring any of the pet's vaccination/medical records to the drop-off appointment.
