This request for a cart delivery is being made as a result of a Republic Services trash and/or recycling cart being stolen or otherwise missing from the residential address.

Please read the form carefully and fill out completely before submitting to the City.

By signing and submitting this form to the City of Lewisville, the resident agrees to the following:

1. Trash and recycling carts are the property of Republic Services.
2. Residents shall not remove Republic Services carts from the address to which the carts are assigned.
3. The resident has conducted a search for the missing cart.
4. If resident fails to report the cart as stolen to the Lewisville Police Department using the online form or calling 972.219.3640, resident must pay a one-time fee of $72.06 for a replacement cart. Failure to include the LPD number assigned to the report (*see below) may also result in the charge being assessed to the water account of the resident.
